Eccovia Solutions Rolls Out PRAPARE SDoH Screening Tool on its Care Coordination Platform

The PRAPARE SDoH screening tool incorporates one of the leading protocols nationwide to measure risk factors that negatively impact at-risk populations. This assessment includes a total of 16 social determinant measures, such as housing stability, transportation, employment status, and social support—and helps community health centers and other providers consider how to treat the person instead of simply reacting to emergencies.

The NACHC screening, known as Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE), is one of the leading SDoH tools nationwide, according to the American Academy of Family Physicians.*
