Collective Medical, delivering the leading ADT-based network for real-time care collaboration today announced that Dr. Deborah Jean Parsons, director of integrated care at Aspire Health Alliance, and Dr. Ronn Berrol, medical director, Summit/Oakland Campus, Alta Bates Summit Medical Center, will be presenting results from respective care collaboration programs at HIMSS20.

Dr. Parsons will present “Critical Connections: Finding and Linking Medicaid Patients to Healthcare,” Tuesday, March 10, 2020, 3:00pm EST in W209C of the Orange County Convention Center. In this session, Dr. Parsons will discuss the unique population health challenges facing providers who manage Medicaid beneficiaries with complex physical, behavioral and mental health needs and explain how real-time updates and care collaboration can effectively and efficiently help deliver complex, high-risk populations the diverse health services they require.

According to an article by Healthcare IT News, use of the Collective platform has helped Aspire case managers track patients to schedule necessary follow-up appointments, ultimately contributing to a 50 percent engagement rate in Aspire’s behavioral health program.

“Aspire participates in MassHealth, our state’s Medicaid system, and we were under some pressure to improve patient outcomes and to reduce our overall costs as part of MassHealth’s plan to streamline patient care,” says Dr. Parsons. “Real-time notifications are so important to help you find and engage with all of your patients—even those with social determinants of health like transient populations or the homeless.”

More than half a million Americans are homeless, and the impact is largely being felt in hospital emergency departments across the country. New state laws demand hospital EDs to take steps to more safely discharge homeless patients but identifying these patients can be difficult when they are often reluctant to disclose their lack of housing.

Dr. Berrol’s presentation “Navigating America’s Hidden Healthcare Crisis: Homelessness”—Wednesday, March 11, 2020, 1:00pm EST in room W330A of the Orange County Convention Center—will explore how to use the resources available in your community and care collaboration tools to make a meaningful impact on patients’ lives, rather than simply being compliant with the law.

“We’ve been using the Collective platform to address the underlying social determinants of health of patients with patterns of high utilization,” says Dr. Berrol. “Many of our patients are struggling with homelessness, so promptly identifying this high-risk population in a busy ED can be problematic. Living conditions greatly effect a patient’s overall health, so knowing that info is critical when developing and coordinating care plans.”

As part of a rapid expansion following the announcement of its Kleiner Perkins-led Series A funding round, Collective now connects patient data across all 50 states. Physicians, nurses and other care team members benefit from patient insights sourced from every state in the nation. Care teams across all points of care are served by the Collective platform, including those based in hospitals, ACOs, emergency departments, physician practices, long-term and post-acute care facilities and behavioral health practices.
